Empty Weight and Balance Work Sheet?

Hi All,
Hope spring has sprung in your neck of the woods, Here in the Pacific Northwest we're tired of the cold and wet.
Question
Does anybody have the work sheet that describes the process of working up the empty weight and balance of a M5-210c, I seem to remember seeing one but for the life of me can't find it.
